****AGREEING TO THIS WAIVER IS NOT A REQUIREMENT TO VOLUNTEER, BUT PLEASE NOTE THAT IT WILL BE YOUR RESPONSIBILITY TO NOTIFY ANYONE AT YOUR VOLUNTEER LOCATION TAKING PICTURES AND/OR RECORDING CONTENT THAT YOU DO NOT WISH FOR YOUR LIKENESS OR VOICE TO BE CAPTURED.****

This Photo/Audio/Informational Assignment and Release Form (“Release”) is executed by and between myself (“Volunteer/Resident/Client”) and Volunteers of America Michigan or any of its entities (“VOAMI.”) I hereby grant to VOAMI, its agents, representatives, and employees the irrevocable,
perpetual, non-exclusive, royalty free and unrestricted right to take photographs of me and my property and audio/video recordings
and story (Collectively, the “Works.”) I grant and convey to VOAMI all right, title, and interest in any and all photographic images,
audio/video recordings and story made by VOAMI without payment or other consideration. This right includes, but is not limited to,
any royalties, proceeds or other benefits arising from or related to any use of the Works.

I agree that VOAMI may use such Works with or without the disclosure of my name. I further agree that VOAMI may
disclose information about me related to the Works with or without my name and for any lawful purpose. Any lawful purpose shall
include, but not be limited to, program promotion, publicity, illustration, advertising and website content. I hereby authorize VOAMI,
its agents, representative and employees to edit, alter, copy, exhibit, publish or distribute the Works in any manner whatsoever. I
hereby waive any right that I may have to inspect and approve any Works or finished products of such Works incorporating my name
or likeness that may be used in connection therewith, or the use to which it may be applied.

I hereby release and forever discharge VOAMI, its agents, representatives and assigns, from any and all claims which arise out
of or are in any way connected to the Works.

This Release extends to, but is not limited to any and all activities, programs and services that I may engage in for VOAMI
and any of its entities now or in the future.

I further agree that in the event that any clause or provision of this document shall be held invalid by any court, the invalidity
of such clause or provision shall not otherwise affect the remaining provisions of this document which shall continue in full force and
effect.

I hereby acknowledge that I have read this Release and fully understand the contents thereof.

Who is this registration for?

STATEMENT OF CONFIDENTIALITY

I, as a volunteer member of Volunteers of America Michigan, do hereby agree to all conditions stated herewith as an integral part of my job description. Any violation of this agreement may result in my immediate dismissal by Volunteers of America Michigan.

I will not discuss or disclose ANY information concerning Volunteers of America Michigan, its employees, operations, clients, financial affairs, or information entrusted to me by an employee, contractor/subcontractor, fellow volunteer, or client outside the confines of Volunteers of America Michigan. I understand that any investigation undertaken on my behalf into confidential files not expressly for the benefit of a client of Volunteers of America Michigan constitutes a breach of the terms of this agreement and may result in immediate termination from Volunteers of America Michigan.

Volunteers of America Michigan reserves the right to investigate any breach of confidentiality fully and to keep all records of said investigation in its files for future reference. Volunteers of America Michigan may require sworn and witnesses documentation by all findings of any investigation and will do everything possible to ensure that the utmost trust and faith in fellow volunteers, employees and contractors/subcontractors is upheld in all possible matters to the extent provided by the law.

Grievances regarding investigations, suspensions, and dismissals for the violations of this agreement will follow the Volunteers of America Michigan Grievance Procedures.

CODE OF ETHICS

All those involved with Volunteers of America are encouraged to pledge to this Code of Ethics as a commitment to achieve the highest possible standards of conduct:

1. I will be conscious of the fact that everything I do, directly or indirectly, has the potential to reflect upon VOA as a whole.

2. I will always conduct myself with Openness, forthrightness, and honesty in dealing with people and organizations, both internally and externally.

3. I will hold myself to the highest possible standard of conduct reflective of the work that I do, striving always to avoid even the appearance of impropriety.

4. I will treat everyone with dignity, worth, respect, concern, courtesy and fairness.

5. I will be sensitive to and seek solutions for all instances of discrimination and social injustice I observe.

6. I will respect and comply with all applicable laws and regulations.

7. I will exercise prudent stewardship of all VOAMI resources.

8. I will avoid conflict-of-interest relationships with board members, staff, suppliers, those we serve and other organizations with whom I deal, unless disclosed and approved.

9. I will accept no gifts or favors, which might influence the performance of my responsibilities.

10. I will also adhere to and comply with the defined standards of conduct of my profession.

LIABILITY WAIVER

I do hereby waive, release and hold harmless Volunteers of America Michigan and its officers and representatives for any injury that may be suffered by myself or the minor child named in this application in the normal course of participation in the designated volunteer activity. Volunteers of America Michigan adheres to strict safety protocols and takes every precaution to minimize the possibility for accidents or injury.
